  • The general discharge disposition ACAH (Advanced Care at Home) is now available in the drop down menu in your HBS DISCHARGE SUMMARY NOTE*
  • Only patients who have been admitted need a discharge summary.  
  • For patients without hospital admit orders, the discharge plan to ACAH should be noted in the H&P/Consult note.
